You may use the provider of your choice (doctor, hospital, lab, etc) nationwide for full benefits. There are no networks for most care (except specialty Rx, transplants).
- Benefits are based on a multiple of Medicare's allowance.
- 150% for outpatient services

- Providers will file your claims. Present your ID card. If your provider is not familiar with "reference based" pricing, they may call Starmark Customer Service at 800-522-1246 Ext. 26300 for answers to questions. Cut out the Wallet Card and keep with your ID card for reference.
- If a provider does not accept the allowed amount, and bills you for charges in excess of the allowed amount, you are not responsible for paying this amount. If you are "balance billed", you should submit a copy of the bill using a form that can be submitted online or emailed or faxed or USPS.
